A formal leave application to a school must include the following parts in order (left-aligned):

1. To Address: The Principal / The Class Teacher, School Name, City.
2. Date: e.g., 15th October 2023.
3. Subject: A brief, one-line reason (e.g., Application for Sick Leave).
4. Salutation: Respected Sir/Madam.
5. Body:
   • Introduction (State who you are: Name, Class, Roll No.)
   • Reason for leave
   • Dates of leave required
6. Complimentary Close: Thanking you, Yours obediently / sincerely.
7. Signature: Your name and class.

Subject: Application for two days' sick leave.
